Out-of-network benefits

If you have out-of-network benefits, contact your insurance company and use the questions below to learn more.

Have your insurance card information available and locate the following:

  • The customer service phone number on the back of your card

  • Your individual ID number and your group number may also be needed

  • Your date of birth

Call your insurance and ask if you have out-of-network acupuncture coverage. If yes, the following information is helpful for you:

  • What date did your insurance coverage begin?

  • Is your plan based on a calendar or fiscal year? If fiscal year, what is the start date?

  • Is your out-of-network acupuncture coverage subject to a deductible? If yes, is there an out-of-network deductible?

  • How much of that deductible remains this year?

  • What is the out-of-network out-of-pocket maximum?

  • How much of the out-of-network out-of-pocket maximum has been met so far this year?

  • Does your plan offer acupuncture benefits?

  • What conditions does the acupuncture benefit cover? (be sure to ask this question as it may not be readily disclosed.)

  • Is your coverage limited to a certain number of visits or a dollar amount each year?

  • How much of the acupuncture benefit has been used so far this year?

If you don’t have out-of-network benefits, be sure to notify your Human Resources representative to request their inclusion in the following year’s insurance plan offerings.
